It's not a rigid plan, but rather a guide that allows you to share your preferences with your healthcare providers. It includes important information about how you envision your labor, delivery, and postpartum experience. 

How to Use It?

1. Fill It Out: We recommend that you complete this document during your third trimester. Take the time to think about your preferences and discuss them with your partner or support person.
2. Review with Your Doctor: Schedule an appointment to review your birth preferences with your healthcare provider. This ensures that your preferences are well-documented and that any questions or concerns are addressed.
3. Bring It to the Hospital: When the time comes, bring a copy of your Birth Preferences Document to the hospital to share with the healthcare team taking care of you during labor and delivery.
